Using Functional Assessment Data to Write and Implement Person-Centered Treatment Plans (PCCP)

Accurate and effective assessments provide the foundation for treatment planning that is person-centered and useful in implementing effective interventions to address consumers’ needs and barriers to achieving goals. For program staff who complete Functional Assessments (including the DLA20), the EBPC offers additional training on using functional assessment rating tools to organize information gathered in assessments, prioritize needs, identify strengths to leverage to overcome barriers to goal achievement, and formulate clearer and more usable treatment plans and supporting documentation.
